Our professional Care Managers are credentialed experts in geriatric care. Our Care Managers work closely with our clients and their families to assess their specific situation and offer guidance through a custom plan of care.
Every family situation and client is unique and that is where the plan of care helps to communicate this information. The plan of care provides day-to-day and long term care goals for the client and provides a convenient document for sharing this information with the client and family. The plan of care is also reviewed with your family member’s Caregiver and our Care Manager on a regular basis to ensure that goals are being met. The plan of care is integral to communication between all parties involved in caregiving for that client.
